Report Incident Form

By simply opening the Cotalker app and filling out the Report Incident form, field workers can send supervisors notifications reporting maintenance issues. After receiving the notification, supervisors have to validate the report.

Fill out the Report Incident form as shown below.

Accessing the Form

To access the form:

• A. Go to the Notifications section.
• B. Press the Action Button.
• C. Select Report Incident to open the survey form.

Form Layout

Once the form appears, proceed as follows:

• D. Fill out the form.
• E. Press Send.

Next step

Once the supervisor receives the notification, they have to validate the report with the Validate Incident form.
